STEP 1: Campaign/Quote Request Form

Download and rename this form to provide the needed information when requesting a paid ads campaign. The completed form will be attached to the Campaign/Quote Work Order.

The fillable PDF is titled FM_Quote_Request.

  • Dowload the form, rename it (replace ‘FM’ with member’s name) and save the form to your desktop or a folder.
  • Open the PDF, fill out all pertinent information, resave the file (with the Member’s name) and attach to Campaign/Quote work order.
  • Send the Campaign/Quote work order to SEO/SEM.
